Questions for your situation
Must the five whys method be used for every problem?
Choose the method according to the problem’s complexity and available data. A simple causal chain can help in some cases, while interacting factors may require other analysis. Demonstrate the causal link rather than completing a fixed question count. Training should include examples showing where different tools are insufficient.
Does writing operator error mean the root cause has been found?
Investigate the conditions that allowed the error to occur and pass undetected. Instructions, equipment, workload, interfaces, training and control points may be relevant. Blaming a person may not reduce recurrence risk. Test the proposed cause against event records and comparative examples where possible, and reopen the analysis if evidence does not support it.
Is cause analysis unnecessary after an urgent correction?
A correction may address the immediate outcome while preventing recurrence still requires examination of causes. Replacing an incorrect label, for example, does not explain how it was selected. Set the depth of analysis according to risk and impact. Training should distinguish temporary containment, correction and lasting action in separate records.
Should only one cause be selected when supplier material and the internal process both contribute?
Several causes and interactions may exist. Examine material batches, process settings, measurements and environmental conditions together. Assigning all responsibility to the supplier can conceal an internal control gap. Include relevant technical parties in the analysis team and identify the evidence required to test each hypothesis.
How many days must pass before an action can be considered effective?
There is no universal number of days. Plan monitoring around recurrence opportunities, production volume, risk and measurable outcomes. Absence of recurrence is weak evidence when insufficient opportunity has arisen. Define before-and-after indicators, acceptance criteria and responsibility for the check.
Can my organisation’s incident report be considered verified at the end of the course?
Course work supports learning and feedback; it differs from formal approval or independent verification of the company’s incident. Use real data only under appropriate authority and confidentiality arrangements. Identify missing evidence and open hypotheses. Check the programme’s conditions for appeals or further work.
