Management statements show only one perspective. Plan discussions with suitable worker roles and shifts to understand practice and participation, including contractor interactions where relevant. Preserve a safe reporting environment. Answers rehearsed from management's script make actual system operation harder to assess.
Does a signed PPE issue record demonstrate that the risk is controlled?
It demonstrates that equipment was issued. First assess eliminating the hazard or using more effective controls. Then examine equipment suitability, correct use, maintenance and worker understanding. An unsuitable machine or process is not adequately controlled merely because protective equipment has been distributed.
Which evidence matters when contractor files are complete but site practices differ?
Investigate the reasons and risks behind differences between approved and actual methods. Assess coordination, supervision and change approval. Records matter, but an unused procedure is not evidence of conformity. Where immediate danger exists, activate arrangements to make work safe; findings cannot be resolved solely by later paperwork.
Is “the worker was careless” an adequate root cause for an accident?
It usually does not explain why the process remained vulnerable to error. Examine work design, equipment, workload, competence, practical instructions and supervision. Identify controls that prevent recurrence rather than automatically assigning personal blame. Assess effectiveness through changed working conditions, not simply the publication of another instruction.
How do different shifts and temporary sites affect audit duration?
Application information should explain activity differences between shifts, temporary locations and critical contractors alongside workforce numbers. Identical work across shifts differs from distinct night maintenance activities. The plan must represent actual operations and risks. Missing information may require the quotation and audit arrangements to be reassessed.
Does ISO 45001 certification remove official occupational safety obligations?
No. Certification assesses the management system within its scope. Statutory appointments, examinations, permits, training and reporting duties continue for the relevant country and activity. A current certificate is not an absolute guarantee of continuous legal compliance. Surveillance and renewal also examine obligation monitoring and responses to significant incidents.
